AUGUSTA, Ga., Sept. 30, 2016

AUGUSTA, Ga., Sept. 30, 2016 /PRNewswire/ -- US CareNet, a national senior care services provider based in Augusta, Ga., today announced that they have purchased controlling interest in the technology used in their NavCare care management division, forming a new company doing business as HC360 Technologies.

"NavCare needed a platform that would address the challenges and continuous changes of healthcare requirements," said Gary Liuzzo, Vice President of Information Technology for US CareNet. "HC360 Technologies is positioned to deliver an advanced, cloud-based solution with integrated Voice over Internet Protocol (VoIP.)"

The newly enhanced platform provides for Chronic Care Management (CCM), Transitional Care Management (TCM) services, and Population Health Management (PHM) services, as well as other care coordination services. In January 2015, the Centers for Medicare and Medicaid Services (CMS) began reimbursing providers who actively manage care delivery for Medicare patients suffering from two or more chronic conditions. These services may be contracted with a third party, such as US CareNet's NavCare division.

About HC360 Technologies

US CareNet formed HC360 Technologies to respond to the ever-evolving healthcare system and its focus on value-based care. Using their more than 30 years of healthcare experience, US CareNet re-engineered and enhanced the platform to more directly address the needs of the chronically ill patient. The company is owned by US CareNet as majority owner and Care 24/7.
