HEART FAILURE NURSE SPECIALIST
PERM, HEADCOUNT ROLE
To deliver the agreed number of clinics at the relevant Trust and ensure that patients are reviewed and managed in line with the pre-agreed initiation and titration protocol
To be responsible for ensuring the smooth running of a safe and effective clinic and service which complies with section 20 regulations of the Health and Social Care Act and is also accountable to the Registered Manager
To work effectively as part of a team to deliver the clinic.
-Ensure that patients are treated with dignity, equality and respect
-Ensure that patients’ questions are answered and patients are given time to consider information
-Ensure patient information is kept confidential
-Deliver safe and effective clinical care
To develop and maintain a level of clinical knowledge to be competent and confident to deliver the specific Heart Failure Review Programme.
-A valid NMC nursing registration
-Pass validations assessments in the required clinical areas
-Demonstrate on-going continual professional development relating specifically to the Heart Failure Programme
