PrescribeWellness, the cloud-based services company whose mission is to inspire collaboration for better health, today announced that it has obtained official phase II certification from Community Care of North Carolina (CCNC) for the Pharmacist eCare Plan initiative. Using PrescribeWellness’ PrescribeCare solution, community pharmacies in North Carolina can share care plans electronically, support care coordination, and actively participate with CCNC and its patient-centered medical home and care-management activities. PrescribeWellness is the first and only pharmacy technology provider to be phase II vendor-certified.

“This phase II certification means PrescribeWellness pharmacies can transmit patient medication lists, drug therapy problems, interventions and patient-centered goals,” said Al Babbington, CEO of PrescribeWellness. “Using PrescribeCare, community pharmacists have the tools they need to go ‘beyond the fill,’ improving patient health outcomes and demonstrating that their encounters reduce costs. We’re thrilled to be a part of that.”

"The Pharmacist eCare Plan is a standard endorsed by the Pharmacy Health Information Technology Collaborative (PHIT) and serves as a standardized, interoperable document for exchanging medication-related activities, plans and goals for an individual needing care,” said Shelly Spiro, Executive Director of Pharmacy HIT Collaborative. “We are pleased that the PrescribeWellness team is adopting the use of this new standard."

Using Technology to Improve Care Coordination

PrescribeCare is an electronic pharmacy “care plan” that pharmacists can use to electronically communicate with other care providers, reducing redundant data entry, improving care coordination, and earning community pharmacies compensation for their care interventions.

Community pharmacists can also use the PrescribeWellness technology platform to identify what interventions or services a patient needs and drive patient services like medication reconciliation, adherence monitoring and medication synchronization, immunizations, Medicare plan reviews, patient engagement, communication to the electronic health record to other healthcare providers, and billing for enhanced services.

“Reaching phase two is an important milestone. PrescribeWellness users can now communicate with payers, quality assurance organizations and other care team members in a way that gives depth and context to their pharmacy’s activities,” said Troy Trygstad, vice president, Pharmacy Programs at Community Care of North Carolina. “This is an important step towards community pharmacies’ meaningful integration with the rest of the health care team. Without the eCare plan, it is difficult for pharmacies to participate in the ongoing healthcare system payment reform, and they risk getting left behind.”

“I'm excited to use PrescribeWellness to work with patients for the eCare plans via PrescribeCare,” said Ashley Abode, PharmD, a clinical service coordinator at Realo Discount Drugs, a community pharmacy in New Bern, North Carolina. “I think it will save a lot of time to have everything under one platform. I like the software integration for day-to-day patient encounters and feel like it will provide a higher level of care for my patients."

PrescribeWellness interfaces with over 50 pharmacy management systems to analyze dispensing data in real time, giving the pharmacy team insight into the needs of their patients and streamlining the medication reconciliation process. Community pharmacies that are not current customers of PrescribeWellness can purchase PrescribeCare as a standalone platform. For current PrescribeWellness customers, PrescribeCare is available for no additional cost.

Collaboration with Community Care of North Carolina (CCNC)

PrescribeWellness developed PrescribeCare to support CCNC and other state Community Pharmacy Enhanced Services Network (CPESN) organizations in their care coordination and patient care initiatives, a network that provides targeted services to an eligible population of more than three million Medicaid, Medicare and dually-eligible recipients. PrescribeCare supports the CCNC CMS Centers for Medicare and Medicaid Innovation project.

“This certification speaks to our continued commitment to supporting projects like CCNC that expand the role of pharmacy, create new payment models, and drive a patient-centric workflow,” added Babbington.
