Primary bacterial resistance to macrolides occurs due to the post-translational methylation of 23s bacterial rRNA. Because of mutation, the bacterial resistance is either plasmid-mediated or chromosomal. This results in the cross-resistance of macrolides, streptogramins, and lincosamides.
